Project Senior Care is partnering with healthcare organizations to provide NO-COST in-home Virtual Care Giving to battle against chronic hospital readmissions.
Our program provides you with a dedicated remote care coordinator who provides proactive check-ins, daily wellness reports, and companionship to combat loneliness. Our 24/7 staff can help with obtaining medications, food, transportation and other household needs. Our program can help with the reasons a senior can end up back in the emergency room.

Service Commencement: For the selected participant, the Home Care Tech Program will begin on the 1st of the following month.
Option to Continue: After 90 days of free service, you’ll have the option to continue at a subsidized price for those who choose to keep the technology going beyond the initial test period.
